Statement regarding LeDeR

LeDeR has confirmed that the causes of death for hundreds of people with learning disabilities were recorded incorrectly, meaning avoidable deaths were underreported. The 2023 findings have now been updated (January 2026), and the picture is even more stark: around 4 in 10 deaths reviewed were classed as avoidable. This is exactly why prevention depends on getting the fundamentals right.
If deaths are misclassified, patterns are missed. Learning is delayed. And families are left waiting for change that should already be happening. SRP exists for one reason: prevention that lands in practice, not hindsight after harm. We cannot normalise this. And we cannot accept a system that corrects the numbers while people keep dying too soon. Full update link in the comments.
